1. Information that may be collected
Only information reasonably needed to arrange, provide, document and administer care is collected. Depending on how you interact with the practice, this may include:
- your name, email address and telephone number;
- the home address where an in-home appointment will take place;
- appointment, service, payment-status and booking-reference information;
- information you provide directly to Scott in connection with assessment, care, documentation or follow-up; and
- limited technical/security information generated when the website is used, where needed to operate and protect the booking system.
The online booking form specifically asks clients not to enter sensitive medical details in the optional booking-note field. Clinical information should be discussed directly with Scott in an appropriate care context.
2. Why information is collected and used
Information is used only for purposes connected to the practice, including responding to inquiries, confirming identity and contact details, scheduling and delivering in-home care, communicating about appointments, processing authorized payments, maintaining appropriate records, protecting the booking system, and meeting professional, tax, legal and regulatory obligations.
3. Consent and choices
When consent is required, it must relate to the information and the purpose for which it is collected, used or disclosed. You may ask questions before providing information and may withdraw consent for future collection, use or disclosure, subject to legal, professional or contractual restrictions and without retroactive effect. Withholding information that is necessary to safely provide or administer care may limit Scott's ability to provide the requested service.
4. Online booking and Square payments
The website uses Square to provide secure card-entry and payment processing. Card numbers, CVV/security codes and card expiry details are entered into Square's payment interface and are not stored in the Scott Foot Care WordPress booking database. The practice stores only the information needed to reconcile the booking and payment, such as the amount, payment status and Square payment identifier.
5. Sharing and disclosure
Personal and personal health information is not sold. Information is disclosed only as reasonably necessary to provide or administer care, when you authorize the disclosure, to service providers acting on the practice's behalf where appropriate safeguards apply, or where disclosure is permitted or required by law. Only the minimum information reasonably necessary for the purpose should be used or disclosed.
6. Safeguards
Reasonable administrative, technical and physical safeguards are used to protect information against loss, theft, unauthorized access, use, disclosure, copying, modification or disposal. Website protections include HTTPS, server-side payment processing, access controls, booking-session validation, rate limiting and limited collection of sensitive information through the public form. No internet system can be guaranteed to be risk-free, so particularly sensitive or urgent clinical information should not be sent through ordinary email or the booking-note field.
7. Retention and secure disposal
Information is retained only for as long as reasonably necessary for care, documentation, payment/accounting, professional, legal and regulatory purposes. When information is no longer required and applicable retention obligations have been met, it is securely destroyed, deleted or de-identified as appropriate.
8. Access and correction
You may ask to access personal health information about you that is under Scott's custody or control, or request a correction where appropriate. Contact Scott using the details below. Identity verification may be required before information is released or changed, and applicable legal exceptions may limit access or correction in some circumstances.
9. Questions, concerns and complaints
Questions or concerns about privacy or information handling should first be directed to Scott. You may also have the right to make a privacy complaint to the Information and Privacy Commissioner of Ontario (IPC). Concerns about a nurse's professional practice can also be addressed through the College of Nurses of Ontario.
10. Website information is not emergency medical advice
This website provides general information and routine booking access. It is not monitored as an emergency service and does not replace urgent assessment. For a new open wound, spreading redness, significant swelling, sudden colour or temperature change, drainage, fever, severe pain or another urgent concern, seek appropriate medical assessment rather than waiting for a routine foot-care appointment.
